What is the C-4 form in Nevada workers' compensation?
The C-4 — the Employee's Claim for Compensation and Report of Initial Treatment — is the form that actually opens your Nevada workers' comp claim. You complete part of it and your treating doctor completes and submits the rest, generally within 3 days of your first visit. The C-4 must be filed within 90 days of the injury, so seeing a doctor promptly matters.
